Using Form Templates

Follow the steps below to create a form using an existing template:

  1. Navigate to the Forms module from the application sidebar and click Create Form.
  2. On the Create Form screen, open the Use Existing Templates tab to access the template gallery.
    • Browse or search templates by form type such as Appointment, Consent, or Registration.
  3. Review template categories from the left panel or use the search bar to find a specific template.
  4. Click Preview to review the structure and fields before selecting a template.
  5. Select the required template and click Use This Template.
    • The form is automatically added to your application.
    • You can modify fields, layout, and logic to suit your business needs.
      Example: An HR manager uses an Employment Application template and customizes it with role-specific questions.
  6. Save and deploy the form to start collecting responses.
